Your first visit to Clark Podiatry Center establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information and give you time to get to know Dr. Macy.
At the time of your first visit, we’ll need to know your medical history. Bring with you a list of all current medications you are taking, including vitamin and herbal supplements. Make sure we’re aware of any allergies you may have to medications, chemicals and foods. Ideally, we like to know how long your condition has existed and what forms of treatment you have attempted, including previous professional treatment.
Please bring with you all insurance identification information. If your insurance requires a copayment at the time of your visit, please bring that with you too. We accept Visa, Mastercard, American Express and Discover.
