We’ll need to know your medical history
Bring with you a list of all current medications you are taking, including vitamin and herbal supplements.
Make sure we’re aware of any allergies you may have to medications, chemicals and foods.
How long your condition has existed and what forms of treatment you have attempted, including previous professional treatment.
Please bring with you all insurance identification information.
If your insurance requires a co-payment at the time of your visit, please bring that with you too. We accept Visa, MasterCard, American Express and Discover.
You can save some time at your first visit by printing out and completing the patient forms in advance of your appointment.
