During the first visit, we make sure to obtain important background information and give you time to get to know Dr. Macy.

  • We’ll need to know your medical history
  • Bring with you a list of all current medications you are taking, including vitamin and herbal supplements.
  • Make sure we’re aware of any allergies you may have to medications, chemicals and foods.
  • How long your condition has existed and what forms of treatment you have attempted, including previous professional treatment.
  • Please bring with you all insurance identification information.
  • If your insurance requires a copayment at the time of your visit, please bring that with you too. We accept Visa, Mastercard, American Express and Discover.
  • You can save some time at your first visit by printing out and completing the patient forms in advance of your appointment.
