A hammertoe is a deformity of the toes, more commonly the 2nd through 5th toes. In this condition, the toe is buckled upward at the middle joint, resulting in deformity and irritation from shoes. Left untreated, hammertoes can become inflexible and require surgery to relieve pain. People with hammertoes may develop corns or calluses on the top of the middle joint of the toe from rubbing on the shoes, between the toes or on the tip of the toe. With or without corns, they may also feel pain in their toes or feet and have difficulty finding comfortable shoes.

Causes of hammertoe deformity are primarily mechanical imbalances.  Shoes that fit poorly can aggravate the problem and cause significant worsening of the pain.

Conservative treatment for the condition typically involves wearing shoes with soft, roomy toe boxes. Debriding corns and padding toes can provide excellent short-term relief.. Commercially available strps or splints rarely produce the straightening of the toes or there relief from pain that is desired.  If using over the counter pads, make sure to use pads wihtout any medication.  Never--I repeat--NEVER use any of the commercial corn removers, drops or medicated disks.  They contain an acid which can damage healthy skin and  can do far worse damage in anybody with compromised sensations or circulation.

In severe cases, hammertoe surgery may be recommended to correct the deformity.  When the deformity is fairly flexible, sometimes a small percutaneous tenotomy can be performed in a few minutes in the office that can provide significant relief with no disability.  Rigid hammertoes can only be corrected bybony surgical correction--arthroplasty and occasionally joint fusion surgery.  Recovery time is relatively rapid from these procedures as well, providing long-lasting relief and reduction of the deformity.
